Ensuring the appropriate management and dispensing of medications for patients is a critical aspect of pharmaceutical care. With Phox, the process of conducting a Frequent Dispensing Assessment has been streamlined to facilitate better patient care. This guide walks you through the step-by-step process.
1. Locate the Patient
First, navigate to the main screen of Phox. Here, you will find the search function prominently located at the top center of the screen. Enter the patient’s first and last name to conduct a thorough search. This initial step is crucial for accurately identifying the patient in question.
2. Access the Patient's Profile
Once the search yields results, sift through to find the desired patient. By clicking on the patient's name, you will be directed to their profile. This section provides a comprehensive overview of the patient’s medical and prescription history.
3. Navigate to the Form Hub
Within the patient's profile, locate the “Hubs” section. Here, you will find various options, but your focus should be on the “Form Hub.” Clicking on this will take you to the next critical phase of the assessment.
4. Initiate Frequent Dispensing Assessment
In the Form Hub, look for the “Frequent Dispensing Assessment” option and select it to start the assessment process. This action opens the assessment form, where you will enter the necessary evaluation details.
5. Select the Patient's Doctor
A list showcasing the patient’s doctors will be displayed, prioritized by the frequency of prescriptions issued. The most frequently prescribing doctor will appear at the top. Select the relevant doctor by clicking the checkbox next to their name, ensuring the assessment is directed appropriately.
6. Pharmacist Assessment
Proceed to the pharmacist assessment section, where you will evaluate the need for a more frequent dispensing interval. This evaluation is based on your professional judgment concerning the patient’s ability to manage their medication, considering any physical, cognitive, or sensory impairments, or a complex medication regimen. Check the boxes that align with your assessment.
7. Articulate Your Rationale
This step involves identifying clinical or safety risks associated with dispensing larger quantities of medication at once. You can select from predefined common reasons or provide a custom rationale. This rationale helps justify the need for a more frequent dispensing schedule.
8. Define the Dispensing Regimen
Select the appropriate dispensing frequency from the provided options. This decision should be based on the rationale provided in the previous step and tailored to the patient’s specific needs.
9. Complete the Signature Section
To authenticate the assessment, conclude the process with your signature. Phox allows for an e-signature or the use of a signature pad, facilitating a seamless process.
10. Submit and Generate the Assessment Form
Upon submission, a completed Frequent Dispensing Assessment form will be generated on Phox. This form includes all details entered during the assessment process, serving as a formal record of the assessment and its outcomes.
Following these steps ensures a thorough and patient-centered approach to determining the necessity for frequent dispensing. This assessment process not only supports better medication management but also enhances overall patient care.
